This application form helps us to reserve the best puppy suited for your family.  Reservations are accepted in order of receipt of this form, and a non-refundable deposit of $500.00 will be required when your application is approved. Your deposit will be applied to the total purchase price ($2,950 + 6.35% sales tax) of the puppy.
